We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Locality Team in Middlesbrough. In this pivotal role, you will work collaboratively with individuals and families to promote their well-being, ensuring they receive the necessary support and services to thrive within their communities. You will be responsible for conducting assessments, developing care plans, and facilitating access to resources, while adhering to best practices and legal frameworks.

Your key responsibilities will include:

Conducting comprehensive assessments of individuals' needs and risks. Developing, implementing, and reviewing person-centered care plans. Working collaboratively with multidisciplinary teams and external agencies. Providing advocacy and support to clients and their families. Maintaining accurate records and documentation in compliance with local policies.

The ideal candidate will possess the following essential qualifications and skills:

A recognized Social Work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Experience in adult social work, particularly within a locality team setting. Strong communication and interpersonal skills, with the ability to build rapport with clients. Excellent organizational skills and the ability to manage a varied caseload.
