Key responsibilities include:

Conducting comprehensive assessments to identify client needs and risks. Developing, implementing, and reviewing care plans in collaboration with clients and multidisciplinary teams. Providing direct support and guidance to clients, ensuring they have access to necessary services and resources. Maintaining accurate and up-to-date case records and documentation. Advocating for clients' rights and facilitating their participation in decision-making processes.

To be successful in this role, you must possess the following qualifications and skills:

A recognized social work qualification (e.g., degree in Social Work). Registration with the relevant professional body (e.g., Social Work England). Proven experience in adult social work, particularly with vulnerable populations. Strong communication and interpersonal skills to effectively engage with clients and stakeholders. Ability to work independently and as part of a team, demonstrating excellent organizational skills.
