Join our dedicated team in Bath and North East Somerset as a Qualified Social Worker in the Adult's Locality Team. We are seeking a passionate and skilled individual to support adults in achieving their best possible outcomes. In this role, you will work collaboratively with service users, families, and multi-agency partners to assess needs, develop care plans, and provide ongoing support to vulnerable adults in our community.

Your key responsibilities will include:

  • Conducting comprehensive assessments to identify the needs and strengths of individuals.
  • Developing, implementing, and reviewing person-centered care plans.
  • Providing direct support and guidance to service users and their families.
  • Collaborating with healthcare professionals, community organizations, and other stakeholders to ensure holistic support.
  • Maintaining accurate and up-to-date case records in accordance with local and national standards.
  • Participating in team meetings and contributing to the development of best practices within the team.

To be considered for this role, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Demonstrable experience in adult social work, particularly in a locality setting.
  • Strong assessment and analytical skills.
  • Excellent communication and interpersonal skills, with the ability to build relationships with diverse individuals.
  • Knowledge of relevant legislation and best practices in adult social care.
