Adult's SW - Community LD Team - East Sussex

Join our dedicated Community Learning Disability Team in East Sussex as a Qualified Social Worker, where you will play a vital role in enhancing the lives of adults with learning disabilities. In this position, you will be responsible for assessing the needs of individuals, developing and implementing person-centered care plans, and collaborating with multidisciplinary teams to ensure the best outcomes for our service users.

Your key responsibilities will include:

  • Conducting comprehensive assessments and reviews of service users' needs and support requirements.
  • Developing and implementing tailored support plans that promote independence and well-being.
  • Working closely with families, carers, and other professionals to coordinate services and support.
  • Advocating for the rights and needs of individuals, ensuring their voices are heard in all aspects of care.
  • Maintaining accurate records and documentation in compliance with local policies and procedures.

The successful candidate will possess essential qualifications and skills, including:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, or DipSW).
  • Registered with Social Work England.
  • Experience working with adults with learning disabilities or in a similar field.
  • Strong communication and interpersonal skills, with the ability to build trusting relationships.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
