We are seeking a dedicated and compassionate Adult's Social Worker to join our Duty Team in Bath & North East Somerset. This is an exciting opportunity for an experienced professional to make a significant impact in the lives of adults in need of support. You will play a vital role in assessing needs, planning interventions, and ensuring the delivery of high-quality social care services. Your key responsibilities will include but are not limited to:
Conducting comprehensive assessments of adults’ needs and risks. Developing and implementing care plans in collaboration with service users and their families. Providing ongoing support and monitoring of care packages to ensure effectiveness. Working closely with other professionals, agencies, and community resources to facilitate holistic care. Maintaining accurate and up-to-date case records in accordance with local policies and procedures.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, especially within a duty team environment. Strong assessment and analytical skills.
