Your key responsibilities will include:
Conducting initial assessments and screenings to determine eligibility for services. Providing information and advice to service users and their families regarding available support options. Developing and implementing care plans in collaboration with service users and relevant stakeholders. Maintaining accurate and up-to-date case records in accordance with local policies and procedures. Participating in team meetings and contributing to the continuous improvement of service delivery.To be considered for this role, you must possess the following qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Demonstrated experience in front-line social work, preferably within a similar setting. Strong assessment and analytical skills, with the ability to make sound decisions under pressure.
