Conducting comprehensive assessments to determine the needs and circumstances of adults requiring support. Developing and implementing tailored care plans in collaboration with clients and their families. Providing ongoing support and advocacy to clients, ensuring their voices are heard and respected. Collaborating with multidisciplinary teams, including healthcare providers, community organizations, and other stakeholders. Maintaining accurate and timely documentation of case notes and progress reports.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, CQSW, or equivalent). Current registration with Social Work England. Proven experience in adult services, with a strong understanding of relevant legislation and best practices. Excellent communication and interpersonal skills, with the ability to build rapport with clients from diverse backgrounds. Strong organizational and time-management skills, with the ability to manage a caseload effectively.
