Join our dedicated Case Management Team in Shropshire as a Senior Social Worker, where you will play a pivotal role in supporting individuals and families through complex social challenges. This position offers the opportunity to lead case management efforts, providing guidance and support to junior staff while ensuring that our clients receive the highest standard of care. You will work within a collaborative environment, engaging with various stakeholders to deliver effective and compassionate solutions.
Your key responsibilities will include:
Conducting comprehensive assessments of clients' needs and circumstances. Developing, implementing, and reviewing care plans in partnership with clients and their families. Providing supervision and mentorship to junior social workers and support staff. Collaborating with multidisciplinary teams and external agencies to ensure coordinated support. Maintaining accurate and up-to-date case records in compliance with legal and organizational standards. Advocating for clients’ rights and access to services.The ideal candidate will possess essential qualifications and skills, including:
A recognized Social Work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Significant experience in social work, particularly in case management or related fields. Strong assessment and analytical skills, with the ability to develop effective intervention strategies. Excellent communication and interpersonal skills, with a commitment to teamwork and collaboration.
