Join our dedicated Learning Disabilities Team in Somerset as a Qualified Social Worker, where you will play a pivotal role in supporting adults with learning disabilities to lead fulfilling lives. In this rewarding position, you will be responsible for assessing individual needs, developing personalized care plans, and coordinating services to ensure the highest quality of support. Collaborating with families, healthcare professionals, and community organizations, you will advocate for the rights and well-being of those you serve.

Your key responsibilities will include:

Conducting comprehensive assessments to identify the needs of adults with learning disabilities. Developing, implementing, and reviewing care plans in collaboration with service users and their families. Providing emotional support and guidance to individuals and their families. Working closely with multidisciplinary teams to ensure integrated service delivery. Maintaining accurate and up-to-date case records in compliance with regulatory standards.

To succeed in this role, you will need:

A recognized social work qualification (e.g., BA/MA in Social Work). Current Social Work England registration. Experience working with adults with learning disabilities or similar client groups. Strong assessment and analytical skills, with the ability to develop person-centered care plans. Excellent communication and interpersonal skills, with a compassionate approach to service delivery.
