We are seeking a dedicated and experienced Advanced Practitioner for our Urgent and Emergency Care team in Nottinghamshire. This pivotal role requires a skilled social worker who can demonstrate expertise in managing complex cases and providing high-quality care to individuals in urgent situations. You will be responsible for assessing needs, developing care plans, and collaborating with multidisciplinary teams to ensure the best outcomes for our clients.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals in urgent and emergency situations. Developing, implementing, and reviewing care plans in collaboration with service users and their families. Providing expert guidance and support to junior staff and social work students. Engaging with external agencies and stakeholders to ensure a coordinated approach to care. Maintaining accurate records and documentation in compliance with regulatory standards.To be considered for this role, you must possess the following essential qualifications and skills:
A recognized social work qualification (BA/MA in Social Work or equivalent). Registered with Social Work England. A minimum of three years of post-qualification experience in social work, particularly in urgent and emergency care settings. Strong assessment and analytical skills with the ability to work under pressure.
