Join our dedicated team in North Somerset as an Adult's Social Worker within the Mental Health/Assessment Team. We are seeking a passionate, qualified individual to provide high-quality social work services to adults experiencing mental health challenges. In this role, you will be responsible for conducting comprehensive assessments, developing care plans, and ensuring that individuals receive the support they need to lead fulfilling lives.
Your key responsibilities will include:
Conducting thorough assessments of clients’ needs and circumstances. Developing and implementing person-centered care plans in collaboration with clients and their families. Providing ongoing support and monitoring of clients’ progress. Working closely with multidisciplinary teams, including healthcare professionals, to ensure holistic care. Advocating for clients’ rights and promoting their independence.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience in mental health social work or a related field. Strong assessment and analytical skills.
