We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work Locality Team in Middlesbrough. You will be responsible for conducting assessments, developing care plans, and providing ongoing support to individuals and their families, ensuring that their needs are met in a person-centered manner.

Your key responsibilities will include:

  • Conducting comprehensive assessments of adults in need of social care services.
  • Developing, implementing, and reviewing care plans in collaboration with service users and their families.
  • Providing advice, support, and advocacy to individuals, helping them to navigate the social care system.
  • Collaborating with multidisciplinary teams and external agencies to ensure holistic support for clients.
  • Maintaining accurate records and documentation in compliance with regulatory standards.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized Social Work qualification (BA/MA in Social Work or equivalent).
  • Registration with Social Work England.
  • Proven experience in adult social work, particularly in a locality team setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users.
  • Excellent assessment and analytical skills, with a commitment to safeguarding vulnerable adults.
