We are currently seeking a dedicated and experienced Qualified Social Worker to join our Camden - SP - Multi Disciplinary team. As a member of this team, you will play a crucial role in providing support and intervention to vulnerable individuals and families in the community.

Key responsibilities and duties include:

  • Conducting assessments to determine the needs of service users and develop appropriate care plans
  • Providing direct support and intervention to individuals and families to address safeguarding concerns and promote well-being
  • Collaborating with other professionals within the multi-disciplinary team to ensure coordinated and holistic support
  • Participating in case conferences, reviews, and court proceedings as required
  • Maintaining accurate and up-to-date records of all interactions and interventions

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with a recognized social work qualification
  • Registered with the Health and Care Professions Council (HCPC)
  • Demonstrated experience in working with vulnerable individuals and families, particularly within a multi-disciplinary team setting
  • Strong assessment and analytical skills, with the ability to make evidence-based decisions
  • Excellent communication and interpersonal skills, with the ability to build rapport and trust with service users
