We are currently seeking a dedicated and experienced Adult's Social Worker to join our Learning Disabilities Team in Lewisham. As a vital member of the team, you will be responsible for providing high-quality social work services to adults with learning disabilities, promoting their independence and well-being.

Key responsibilities and duties include:

Assessing the needs of individuals with learning disabilities and developing care plans tailored to their specific requirements. Working collaboratively with multi-disciplinary teams, service users, and their families to ensure holistic support. Advocating for the rights and choices of individuals with learning disabilities, empowering them to live fulfilling lives. Monitoring and reviewing care plans regularly to assess progress and make necessary adjustments. Participating in safeguarding procedures and promoting a safe environment for vulnerable adults.

The ideal candidate for this role will possess:

A recognized Social Work qualification and registration with Social Work England. Demonstrated experience in working with adults with learning disabilities within a social work setting. Strong assessment and communication skills, with the ability to build rapport with service users and stakeholders. An understanding of relevant legislation and policies related to adult social care and safeguarding.
