We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our team in Slough in a Hospital Discharge role. As a key member of our social work team, you will play a crucial role in facilitating safe and timely discharges for patients, ensuring they receive the necessary support and services upon leaving the hospital.

Key responsibilities and duties:

Assessing the needs of patients being discharged from the hospital and developing comprehensive discharge plans. Liaising with healthcare professionals, community services, and external agencies to coordinate post-discharge support. Providing advocacy and support to patients and their families to ensure a smooth transition from hospital to home or alternative care settings. Completing all necessary documentation and reports in a timely and accurate manner. Participating in multi-disciplinary meetings and case conferences to ensure a holistic approach to patient care.

Essential qualifications and skills:

Qualified Social Worker with a valid registration with Social Work England. Demonstrated experience in a similar role, preferably within a hospital or healthcare setting. Strong assessment and case management skills with the ability to work autonomously and as part of a team. Excellent communication and interpersonal skills, with the ability to build rapport with patients and collaborate effectively with colleagues and external partners. Knowledge of relevant legislation and policies related to social work practice in healthcare settings.

Additional requirements:

Full driving license and access to a vehicle for travel within the community. Flexibility to work occasional weekends or evenings as required.
