We are currently seeking a dedicated and compassionate Adult Social Worker specializing in Older People to join our team in Doncaster. In this role, you will be responsible for providing high-quality social work services to support the well-being and independence of older individuals in our community.

Key responsibilities and duties:

Conducting assessments to determine the needs of older people and develop care plans accordingly Providing support and guidance to older individuals and their families to promote their overall welfare Collaborating with healthcare professionals, community resources, and other stakeholders to ensure comprehensive care Advocating for the rights and best interests of older people to safeguard their dignity and autonomy Maintaining accurate and up-to-date records of interventions and progress

Essential qualifications and skills:

Qualified Social Worker with relevant experience in working with older people Registered with the Health and Care Professions Council (HCPC) Strong communication and interpersonal skills to engage effectively with older individuals and their support networks Ability to assess complex situations and make informed decisions in the best interest of older people Knowledge of relevant legislation, policies, and procedures related to adult social care
