Adult's SW - MH Team - Swindon
We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work – Mental Health Team in Swindon. This pivotal role involves working with adults experiencing mental health challenges, providing them with the support and resources necessary to improve their quality of life. As part of a collaborative team, you will be instrumental in delivering high-quality social work services that promote mental well-being and empower individuals to achieve their personal goals.
Your key responsibilities will include:
- Conducting comprehensive assessments of clients’ needs and risks, ensuring that appropriate support plans are developed.
- Working collaboratively with clients, families, and other professionals to facilitate access to mental health services and community resources.
- Providing ongoing case management and support, monitoring progress, and adjusting care plans as necessary.
- Advocating for clients’ rights and needs, ensuring their voices are heard in all aspects of care.
- Maintaining accurate and timely documentation of all interactions and interventions in compliance with regulatory standards.
To be successful in this role, you will need to possess essential qualifications and skills, including:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
- Registration with Social Work England.
- Experience in adult social work, particularly in mental health settings.
- Strong assessment and analytical skills, with the ability to develop effective care plans.
- Excellent communication and interpersonal skills, enabling you to build rapport with clients and work effectively within a multidisciplinary team.
