Are you a dedicated Social Worker looking to make a positive impact in the community? We are seeking a passionate and qualified Social Worker for the Adults Community team in Darlington. In this role, you will work collaboratively with individuals and families to assess needs, develop care plans, and facilitate access to services that enhance the quality of life for adults in the community.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals’ needs and circumstances. Developing, implementing, and reviewing care plans in collaboration with service users and other professionals. Providing information, advice, and support to adults and their families, ensuring they understand their rights and options. Working closely with multidisciplinary teams, including healthcare providers, to coordinate services and ensure holistic support. Maintaining accurate and up-to-date records in compliance with local and national standards.The ideal candidate will possess the following qualifications and skills:
A recognized qualification in Social Work (e.g., Degree in Social Work or equivalent). Registration with Social Work England. Experience working with adults in a community setting, preferably in a social work capacity. Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
