We are currently seeking a dedicated Adult's Social Worker to join our Locality Team in North Lincolnshire. As a key member of the team, you will be responsible for providing high-quality social work services to adults in the local community.

Key responsibilities and duties include:

  • Conducting assessments and developing care plans for adults in need of support
  • Working collaboratively with multi-disciplinary teams to ensure the best outcomes for service users
  • Providing advocacy and support to vulnerable adults to safeguard their well-being
  • Participating in case conferences and reviews to monitor progress and make necessary adjustments to care plans
  • Keeping accurate and up-to-date records of interventions and outcomes

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC)
  • Demonstrated experience in working with adults in a social care setting
  • Strong communication and interpersonal skills to engage effectively with service users and stakeholders
  • Ability to work autonomously and make sound decisions in complex situations
  • Knowledge of relevant legislation and policies related to adult social care
