We are currently seeking a dedicated and passionate Adult's Social Worker to join our Mental Health Team in North Lincolnshire. As a key member of the team, you will be responsible for providing high-quality social work services to adults with mental health needs, ensuring their well-being and promoting their independence.

Key responsibilities and duties:

  • Conducting assessments and developing care plans for individuals with mental health issues
  • Working collaboratively with multidisciplinary teams to provide holistic support
  • Advocating for clients and ensuring their voices are heard in decision-making processes
  • Monitoring and reviewing the effectiveness of interventions and making necessary adjustments
  • Providing crisis intervention and support during times of acute mental health distress

Essential qualifications and skills:

  • Qualified Social Worker with current registration with HCPC
  • Demonstrated experience working with adults with mental health needs
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively under pressure and manage complex cases
