Your key responsibilities will include:

Conducting comprehensive assessments of individuals' needs and risks. Developing and implementing tailored care plans in accordance with legislation and best practices. Maintaining accurate and up-to-date case records and documentation. Engaging with service users, families, and other professionals to ensure effective service delivery. Participating in team meetings and contributing to service development initiatives.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience in adult social work, particularly in assessment and care planning. Strong communication and interpersonal skills, with the ability to build rapport with individuals and families. Excellent organizational skills and the ability to manage a caseload effectively.
