We are seeking a dedicated and compassionate Adults Social Worker to join our Community Team in North Lincolnshire. Your key responsibilities will include:

Conducting comprehensive assessments of adults’ social care needs. Developing and implementing personalized care plans in collaboration with service users and their families. Providing ongoing support and advocacy to ensure individuals receive the appropriate services and resources. Working closely with other professionals, agencies, and community resources to facilitate effective service delivery. Maintaining accurate and up-to-date case records in line with legislative requirements.

To be successful in this role, you will need to possess the following essential qualifications and skills:

A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Strong knowledge of legislation, policies, and best practices in adult social care. Excellent interpersonal and communication skills, with the ability to build rapport with service users. Proven experience in working with vulnerable adults and a commitment to promoting their rights and well-being.
