We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Multi-Disciplinary Team in Wirral. This pivotal role involves working collaboratively with various professionals to provide high-quality support and interventions for adults with complex needs. You will play a crucial part in assessing, planning, and implementing care packages that promote independence and enhance the quality of life for our service users.

Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals' needs and risks.
  • Developing and reviewing care plans in partnership with service users and their families.
  • Collaborating with health services, housing, and other community resources to ensure holistic support.
  • Providing advice, guidance, and support to service users and their families.
  • Maintaining accurate records and documentation in line with statutory requirements.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Strong understanding of adult safeguarding, mental capacity, and relevant legislation.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team.
  • Proven experience in adult social work, ideally within a multi-disciplinary setting.
