We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Multi-Disciplinary Team in Wirral. This pivotal role involves working collaboratively with various professionals to provide high-quality support and interventions for adults with complex needs. You will play a crucial part in assessing, planning, and implementing care packages that promote independence and enhance the quality of life for our service users.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and risks. Developing and reviewing care plans in partnership with service users and their families. Collaborating with health services, housing, and other community resources to ensure holistic support. Providing advice, guidance, and support to service users and their families. Maintaining accurate records and documentation in line with statutory requirements.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Strong understanding of adult safeguarding, mental capacity, and relevant legislation. Excellent communication and interpersonal skills, with the ability to work effectively in a team.
