The London Borough of Harrow is seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work - Learning Disabilities/Assessment Team. This role is pivotal in supporting adults with learning disabilities, ensuring they receive the necessary assessments and interventions to enhance their quality of life. As a key member of our team, you will work collaboratively with service users, families, and other professionals to deliver person-centered care and support.

Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and strengths.
  • Developing and implementing care plans in collaboration with service users and their families.
  • Providing ongoing support and advocacy for service users to promote independence and well-being.
  • Working closely with multidisciplinary teams to ensure a holistic approach to care.
  • Maintaining accurate and up-to-date case records in accordance with local policies and procedures.

To be considered for this position, you must possess the following qualifications and skills:

  • A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Experience working with adults with learning disabilities or in a similar social work setting.
  • Strong assessment and analytical skills, with the ability to develop effective care plans.
  • Excellent communication and interpersonal skills, with a commitment to working collaboratively.
