Join our dedicated team in South Gloucestershire as a Qualified Social Worker specializing in Hospital Discharge. In this vital role, you will work closely with patients, families, and healthcare professionals to ensure smooth transitions from hospital to home, promoting independence and well-being. Your expertise will be crucial in assessing needs, coordinating care, and implementing personalized discharge plans that prioritize patient safety and satisfaction.
Key responsibilities include:
Conducting comprehensive assessments of patients' social, emotional, and practical needs prior to discharge. Collaborating with multidisciplinary teams to develop and implement effective discharge plans. Providing support and guidance to patients and their families throughout the discharge process. Ensuring compliance with relevant policies, procedures, and legislation. Maintaining accurate and timely records of assessments, interventions, and outcomes.To be successful in this role, you must possess the following qualifications and skills:
A recognized Social Work qualification (BA/MA in Social Work or equivalent). Registration with Social Work England. Proven experience in hospital discharge or similar settings. Strong communication, interpersonal, and organizational skills.
