We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Learning Disabilities (LD) Team in Swindon. This role is pivotal in supporting adults with learning disabilities to lead fulfilling lives and achieve their personal goals. You will work collaboratively with individuals, families, and other professionals to provide high-quality social care services tailored to the needs of our clients.

Your key responsibilities will include:

  • Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities.
  • Developing and implementing person-centered care plans in collaboration with clients and their families.
  • Providing ongoing support, guidance, and advocacy to clients to enhance their independence and quality of life.
  • Working closely with multidisciplinary teams, including health professionals and community organizations, to ensure holistic care.
  • Maintaining accurate records and documentation in compliance with legal and organizational standards.

The ideal candidate will possess the following qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
  • Current registration with Social Work England.
  • Proven experience in adult social work, particularly with learning disabilities.
  • Strong assessment and analytical skills.
  • Excellent communication and interpersonal skills, with a commitment to empowering clients.
