We are currently seeking a dedicated and experienced Qualified Social Worker to join our Post Order and Permanence Team in Rochdale. As a key member of the team, you will be responsible for managing cases post-order, ensuring the permanence and stability of placements for children in need.
Key responsibilities and duties include:
Conducting comprehensive assessments of children and families to determine their needs and support requirements Developing and implementing care plans in collaboration with stakeholders to promote the well-being and safety of children Providing ongoing support and guidance to children in care, foster families, and adoptive parents Attending court hearings and reviews to represent the best interests of the children Working closely with other professionals, such as health care providers and educators, to ensure holistic support for childrenThe ideal candidate for this role will possess:

We are seeking a dedicated and experienced Qualified Social Worker to join our Adults Mental Health Team. You will be responsible for delivering high-quality, person-centred support to adults experiencing mental health challenges, ensuring statutory responsibilities are met in line with the Care Act 2014, Mental Health Act 1983, and other relevant legislation.
Key Responsibilities:Manage a caseload of adults with a range of complex mental health needs, including those subject to the Care Programme Approach (CPA).Undertake strength-based assessments and develop care and support plans that promote independence and recovery.Work collaboratively with health professionals, including psychiatrists, community psychiatric nurses, and psychologists, as part of a multidisciplinary team.Carry out safeguarding enquiries under Section 42 of the Care Act.Participate in Mental Health Act assessments and provide aftercare under Section 117, where appropriate.Support service users in accessing appropriate community resources, services, and accommodation.Maintain accurate and up-to-date case records in line with departmental policies and professional standards.Attend and contribute to team meetings, case conferences, and strategy discussions.Comply with all relevant legislation, policies, procedures, and professional codes of practice.Requirements:Social Work qualification (BA/MA Social Work, DipSW, or equivalent).Registration with Social Work England.Proven post-qualification experience within an adults' mental health setting.Sound knowledge of relevant legislation (Care Act 2014, Mental Health Act 1983, Mental Capacity Act 2005, Human Rights Act).Ability to work independently and as part of a team in a dynamic environment.Excellent communication, assessment, and report writing skills.Enhanced DBS check (within 12 months or on the update service).Desirable:AMHP (Approved Mental Health Professional) status or willingness to train.Experience working in a local authority or NHS mental health service.Familiarity with LiquidLogic or similar case management systems.
Qualified Social Worker with evidence of relevant qualifications
Minimum of 5 years post qualifying experience
Registered with Social Work England
Experience of working in an Adult Social Care statutory setting
Knowledge and experience of working with people who have severe and enduring mental ill-health
Experience of undertaking assessments of need, undertaking scheduled and unscheduled reviews, creating care-plans and commissioning support to meet identified needs.
Experience of statutory safeguarding under s42(1) and s42(2) Care Act
Experience of completing Mental Capacity Assessments
Knowledge and understanding of the Care Act, Mental Capacity Act, Mental Health Act, Equalities Act, Human Rights Act, Data protection and GDPR, Continuing Health Care.
Knowledge and understanding of Deprivation of Liberty Safeguards.

The Job:
Due to career progressions, Agency Social Worker opportunities are now available within the the Mental Health Review Team in Middleton and CMHT West in Rochdale. The role is to undertake social work statutory functions within an Adult Care mental health service. We are co-located with health colleagues and work in a multi-disciplinary way.
We require agency social workers to undertake Strength Based Needs assessments, review packages of care, update care plans, undertake mental capacity assessments where appropriate, undertake safeguarding work where appropriate and facilitate hospital discharges for people who have a severe and enduring mental health diagnosis and who are under secondary mental health services. Individuals may live within the borough of Rochdale or may be placed out of borough. It will be helpful to have an understanding of mental health pathways.
WORKING PATTERN:
37 hours per week
Monday to Friday
There is some scope to work from home with manager agreement, however the role is primarily office based due to the needs of the client group
