Join our dedicated team in Lancashire as a Social Worker in Acute Adults Services, where you will play a crucial role in supporting vulnerable adults in crisis. In this challenging yet rewarding position, you will work collaboratively with multidisciplinary teams to assess, plan, and implement effective interventions that promote the wellbeing and safety of individuals in acute settings. Your expertise will be vital in ensuring that individuals receive the appropriate support and resources they need during critical times.
Your key responsibilities will include:
Conducting thorough assessments of clients' needs, risks, and strengths. Developing and implementing personalized care plans in collaboration with clients and their families. Providing crisis intervention and support to individuals experiencing acute mental health issues. Maintaining accurate and up-to-date case records and documentation. Working closely with healthcare professionals and community organizations to facilitate seamless transitions of care. Advocating for clients’ rights and ensuring their voices are heard in decision-making processes.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Experience in adult social work, particularly in acute or crisis settings. Strong assessment and intervention skills, with a person-centered approach. Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
