We are seeking a dedicated and experienced Qualified Social Worker to join our Adult's Social Work – Learning Disabilities Team in Leeds. This role offers a unique opportunity to make a positive impact on the lives of adults with learning disabilities, working collaboratively with individuals, families, and multidisciplinary teams to promote independence and enhance quality of life.
Key responsibilities include:
Conducting comprehensive assessments of individuals with learning disabilities to identify their needs, strengths, and aspirations. Developing and implementing personalized support plans in collaboration with service users and their families. Providing ongoing support and advocacy to individuals, ensuring their voices are heard in decision-making processes. Working closely with healthcare professionals, community organizations, and other stakeholders to coordinate services and resources. Maintaining accurate and up-to-date case records and documentation in compliance with local and national guidelines.The ideal candidate will possess the following essential qualifications and skills:
A relevant Social Work qualification (Degree in Social Work or equivalent). Registered with Social Work England. Demonstrable experience in adult social work, particularly within learning disabilities. Strong assessment and intervention skills, with a person-centered approach.
