We are currently seeking a dedicated and experienced Adult's Social Worker to join our Mental Health Team in Wiltshire. As a key member of the team, you will be responsible for providing high-quality social work services to adults with mental health needs, ensuring their well-being and promoting independence.

Key responsibilities and duties:

  • Assessing the needs of adults with mental health issues and developing care plans
  • Providing support and guidance to service users and their families
  • Liaising with healthcare professionals, community resources, and other agencies to coordinate care
  • Advocating for the rights and best interests of service users
  • Maintaining accurate and up-to-date case records and reports

Essential qualifications and skills:

  • Qualified Social Worker with a relevant degree and HCPC registration
  • Experience working with adults with mental health issues in a social work setting
  • Strong assessment, communication, and decision-making skills
  • Ability to work effectively in a multi-disciplinary team
  • Knowledge of relevant legislation and policies related to adult social care and mental health
