This role is pivotal in supporting young adults as they transition from youth services to independent living. You will work closely with individuals aged 16 to 25, providing guidance and support to help them navigate the challenges of adulthood while promoting their well-being and personal development.
Your key responsibilities will include:
- Assessing the needs of young adults and developing tailored support plans.
- Building trusting relationships with clients and their families to ensure effective communication and collaboration.
- Coordinating with various agencies and professionals to provide holistic support.
- Monitoring progress and reviewing support plans regularly to ensure the best outcomes for clients.
- Providing advocacy and support during meetings and appointments, empowering young adults to voice their needs and aspirations.
The ideal candidate will possess essential qualifications and skills, including:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
- Registration with Social Work England.
