We are currently seeking an experienced and dedicated Social Worker to join our team in Oldham in a peripatetic position within the Qualified Social Work field. As a key member of our team, you will play a crucial role in supporting and advocating for vulnerable individuals and families in the community.
Key responsibilities and duties:
- Conducting assessments and developing care plans for individuals in need of support
- Providing direct social work intervention and support to clients
- Collaborating with other professionals and agencies to ensure holistic care for clients
- Participating in case conferences and reviews to monitor progress and make necessary adjustments to care plans
- Maintaining accurate and up-to-date records of client interactions and progress
Essential qualifications and skills:
- Qualified Social Worker with relevant experience in a similar role
- Registration with Social Work England
- Strong communication and interpersonal skills
- Ability to work effectively in a multi-disciplinary team
- Excellent assessment and decision-making skills
