We are currently seeking a dedicated and passionate Adult's Social Worker to join our Learning Disabilities Team in Warwickshire. As a key member of the team, you will be responsible for providing high-quality support and services to adults with learning disabilities, ensuring their well-being and promoting their independence.
Key responsibilities and duties:
- Conducting assessments and developing care plans for individuals with learning disabilities.
- Working collaboratively with other professionals and agencies to provide a comprehensive support network.
- Advocating for clients and ensuring their voices are heard in decision-making processes.
- Monitoring and reviewing the progress of individuals, adjusting support plans as needed.
- Providing advice and guidance to families and caregivers on best practices for supporting individuals with learning disabilities.
Essential qualifications and skills:
- Qualified Social Worker with a valid registration with Social Work England.
- Demonstrated experience working with adults with learning disabilities in a social work setting.
- Strong assessment and care planning skills, with the ability to tailor support to individual needs.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients and colleagues.
- Ability to work effectively in a multidisciplinary team and collaborate with external partners.
