We are currently seeking a dedicated and passionate Adult's Social Worker to join our Learning Disabilities Team in Warwickshire. As a key member of the team, you will be responsible for providing high-quality support and services to adults with learning disabilities, ensuring their well-being and promoting their independence.

Key responsibilities and duties:

Conducting assessments and developing care plans for individuals with learning disabilities. Working collaboratively with other professionals and agencies to provide a comprehensive support network. Advocating for clients and ensuring their voices are heard in decision-making processes. Monitoring and reviewing the progress of individuals, adjusting support plans as needed. Providing advice and guidance to families and caregivers on best practices for supporting individuals with learning disabilities.

Essential qualifications and skills:

Qualified Social Worker with a valid registration with Social Work England. Demonstrated experience working with adults with learning disabilities in a social work setting. Strong assessment and care planning skills, with the ability to tailor support to individual needs. Excellent communication and interpersonal skills, with the ability to build rapport with clients and colleagues. Ability to work effectively in a multidisciplinary team and collaborate with external partners.

We are currently seeking a dedicated and experienced Qualified Social Worker to join our Warwickshire Adults Learning Disability Team. As a key member of the team, you will be responsible for providing high-quality social work services to adults with learning disabilities, ensuring their well-being and promoting their independence.

Key responsibilities and duties include:

Conducting assessments to identify the needs and strengths of individuals with learning disabilities Developing and implementing care plans in collaboration with service users, their families, and other professionals Providing support and guidance to individuals to enhance their social functioning and quality of life Advocating for the rights and interests of adults with learning disabilities Working closely with multi-disciplinary teams to coordinate services and support for service users

The ideal candidate for this position will possess:

A recognized social work qualification and registration with Social Work England Demonstrated experience working with adults with learning disabilities in a social work setting Strong assessment and care planning skills Excellent communication and interpersonal skills Ability to work effectively both independently and as part of a team
