We are seeking a dedicated and compassionate Qualified Social Worker (QSW) specializing in Learning Disabilities and Transitions to join our dynamic team in Warwickshire. As a vital member of our Allied Health Professionals (AHP) field, you will play a crucial role in supporting individuals with learning disabilities as they navigate the transition to adulthood and independent living. Your expertise will help empower clients and their families, ensuring they receive the best possible care and support.

Key responsibilities for this position include:

Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities. Developing and implementing personalized care plans in collaboration with clients, families, and multi-disciplinary teams. Providing ongoing support and guidance to clients during their transition periods, ensuring a smooth and positive experience. Advocating for clients’ rights and ensuring they have access to necessary services and resources. Maintaining accurate and timely documentation in compliance with regulatory standards.

To be considered for this role, candidates must possess the following qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, CQSW, CSS, or equivalent). Registration with Social Work England. Experience working with individuals with learning disabilities and knowledge of transition processes. Strong communication and interpersonal skills, with the ability to empathize and connect with clients and their families. Excellent organizational skills and the ability to manage a diverse caseload effectively.
