We are currently seeking a dedicated and experienced Adult's Social Worker to join our South Community Team in Bristol. As a key member of our team, you will be responsible for providing high-quality social work services to adults in need, with a focus on promoting independence and enhancing the well-being of our service users.

Key responsibilities and duties include:

  • Assessing the needs and risks of adults in the community
  • Developing and implementing care plans in collaboration with service users and their families
  • Providing support and guidance to service users to empower them to make informed decisions
  • Working closely with multi-disciplinary teams and external agencies to ensure holistic care
  • Participating in safeguarding procedures and advocating for the rights of vulnerable adults

The ideal candidate for this role will possess the following essential qualifications and skills:

  • Qualified Social Worker with a relevant degree and HCPC registration
  • Demonstrated experience in adult social work, preferably within a community setting
  • Strong assessment and case management skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively under pressure and manage competing priorities

In addition to the above requirements, candidates with experience in mental health, learning disabilities, or older adults' services will be highly regarded.
