We are currently seeking a dedicated and experienced Qualified Social Worker to join our Westminster - QSW - South Complex Care team. As a key member of the team, you will be responsible for providing high-quality social work services to individuals with complex care needs.

Key responsibilities and duties include:

  • Conducting comprehensive assessments to identify the needs of service users
  • Developing and implementing care plans in collaboration with multi-disciplinary teams
  • Providing ongoing support and intervention to promote the well-being and independence of service users
  • Advocating for the rights and best interests of vulnerable individuals
  • Participating in case conferences and reviews to ensure the effectiveness of care plans

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC)
  • Demonstrated experience working with individuals with complex care needs, preferably in a similar setting
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and colleagues
  • Excellent assessment and analytical skills to identify needs and develop appropriate interventions
  • Ability to work effectively in a fast-paced and challenging environment, prioritizing workload and meeting deadlines
