We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Learning Disabilities & Autism Team in Telford. This role is pivotal in providing high-quality support and intervention for adults with learning disabilities and autism, ensuring they lead fulfilling lives within their communities. As part of our dynamic team, you will work collaboratively with individuals, families, and other professionals to create tailored support plans that promote independence and well-being.

Your key responsibilities will include:

  • Conducting assessments to identify the needs and strengths of individuals with learning disabilities and autism.
  • Developing and implementing person-centered support plans in collaboration with service users and their families.
  • Providing ongoing support, guidance, and advocacy to individuals and their families.
  • Working closely with multi-disciplinary teams to ensure holistic care and support.
  • Maintaining accurate and up-to-date records in compliance with relevant legislation and policies.

To be considered for this role, you must possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work or equivalent).
  • Registration with Social Work England.
  • Experience working with adults with learning disabilities and autism.
  • Strong assessment and analytical skills.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
