As a key member of our team, your responsibilities will include:

Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities and autism. Developing and implementing person-centered support plans that promote independence and well-being. Providing direct support and advocacy to service users, ensuring their voices are heard in decision-making processes. Collaborating with multidisciplinary teams, including health professionals, to ensure holistic care and support. Maintaining accurate case records and documentation in compliance with statutory requirements.

The ideal candidate will possess the following qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Proven experience working with adults with learning disabilities and autism. Strong assessment and analytical skills, with the ability to develop effective support plans. Excellent communication and interpersonal skills, with a compassionate approach to service users.
