Join our dedicated team at Southampton City Council as an Adults Social Worker within the Connect Team. In this role, you will be responsible for assessing the needs of individuals, developing care plans, and ensuring that services are delivered effectively and efficiently. Your expertise will play a vital role in promoting independence and enhancing the quality of life for vulnerable adults.
Key responsibilities include:
Conducting comprehensive assessments to identify the needs and preferences of service users. Developing and implementing care plans in collaboration with individuals, families, and other professionals. Providing ongoing support, guidance, and advocacy to service users and their families. Monitoring and reviewing care plans to ensure they meet the changing needs of service users. Working closely with multidisciplinary teams and external agencies to coordinate services and support.To be successful in this role, you will need the following qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly within a community setting. Strong assessment and analytical skills, with the ability to develop tailored care plans.
