Join our dedicated team as a Social Worker within the Adult's SW - MIT Team in Sheffield, where you will play a vital role in supporting adults with complex needs. Your primary responsibilities will include conducting assessments, developing care plans, and delivering high-quality support services to individuals and families. You will work collaboratively with multi-disciplinary teams to ensure the best outcomes for our service users, advocating for their rights and well-being.

Key responsibilities include:

Conducting comprehensive assessments of service users’ needs and circumstances. Developing, implementing, and reviewing care plans in partnership with service users. Providing advice, support, and guidance to individuals and their families. Maintaining accurate and up-to-date records in compliance with legal and organizational standards. Participating in team meetings and contributing to service development initiatives.

To be considered for this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., CQSW, CSS, or equivalent). Registration with Social Work England. Demonstrated experience in adult social work, particularly with vulnerable populations. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals. Excellent organizational skills and the ability to manage competing priorities effectively.
