Your key responsibilities will include:

Conducting comprehensive assessments to identify the needs and strengths of service users. Creating and implementing person-centered care plans that promote individual goals. Maintaining accurate and up-to-date case records and documentation. Providing ongoing support and advocacy for service users and their families. Collaborating with multidisciplinary teams to ensure integrated service delivery.

To be successful in this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England. Strong understanding of relevant legislation and best practices in adult social care. Excellent communication and interpersonal skills. Ability to work effectively under pressure and manage a varied caseload.
