We are seeking a dedicated and passionate Qualified Social Worker (QSW) to join our Transitions Team in West Sussex. This is an exciting opportunity to play a pivotal role in supporting young people as they transition into adulthood. As part of our dynamic team, you will work collaboratively with children, families, and other professionals to promote positive outcomes and ensure the best possible support for our service users.
Your key responsibilities will include:
- Conducting comprehensive assessments of young people's needs and capabilities.
- Developing and implementing tailored support plans that facilitate smooth transitions into adulthood.
- Working closely with families, educators, and other stakeholders to coordinate services and resources.
- Providing advocacy and guidance to young people, empowering them to make informed decisions.
- Monitoring and reviewing progress, making necessary adjustments to support plans as required.
To be successful in this role, you must possess the following qualifications and skills:
- A recognized social work qualification (e.g., CQSW, DipSW, or equivalent).
- Current registration with Social Work England.
- Experience working with young people, particularly in transition planning.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent organizational and time management abilities, with a proactive approach to problem-solving.
