We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Calderdale. As a key member of our multidisciplinary team, you will work collaboratively to assess, plan, and implement high-quality support for adults facing various challenges related to health and social care.

Your key responsibilities will include:

Conducting comprehensive assessments of individuals’ needs and risks in a hospital setting. Developing and implementing care plans in collaboration with service users, families, and healthcare professionals. Providing emotional and practical support to individuals and their families during hospital stays and transitions to home or community care. Ensuring compliance with relevant legislation, policies, and best practices in social work. Maintaining accurate and up-to-date records of assessments, interventions, and outcomes.

To be considered for this role, you must possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, preferably in a hospital or healthcare setting. Strong assessment and analytical skills, with the ability to manage complex cases. Excellent communication and interpersonal skills to engage effectively with service users and stakeholders.
