We are seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Hospital Discharge Team in St Helens, offering a competitive rate of £35 per hour. This role is pivotal in facilitating smooth transitions for patients from hospital to home, ensuring their ongoing care needs are met effectively and compassionately. You will work closely with patients, families, and multidisciplinary teams to assess needs, plan safe discharges, and coordinate necessary support services.

Key responsibilities include:

  • Conducting comprehensive assessments of patients’ social care needs upon discharge.
  • Developing and implementing discharge plans in collaboration with patients, families, and healthcare professionals.
  • Providing information and guidance on available community resources and support services.
  • Ensuring compliance with local and national policies regarding discharge protocols.
  • Maintaining accurate and timely documentation of assessments and discharge plans.

To be considered for this role, candidates must possess the following qualifications and skills:
