Join our dedicated Community Learning Disability Team in East Sussex as a Qualified Social Worker, where you will play a vital role in enhancing the lives of adults with learning disabilities. In this position, you will be responsible for assessing the needs of individuals, developing and implementing person-centered care plans, and collaborating with multidisciplinary teams to ensure the best outcomes for our service users.
Your key responsibilities will include:
Conducting comprehensive assessments and reviews of service users' needs and support requirements. Developing and implementing tailored support plans that promote independence and well-being. Working closely with families, carers, and other professionals to coordinate services and support. Advocating for the rights and needs of individuals, ensuring their voices are heard in all aspects of care. Maintaining accurate records and documentation in compliance with local policies and procedures.The successful candidate will possess essential qualifications and skills, including:
A recognized social work qualification (e.g., Degree in Social Work, CQSW, or DipSW). Registered with Social Work England. Experience working with adults with learning disabilities or in a similar field. Strong communication and interpersonal skills, with the ability to build trusting relationships.
