An experienced Adult Social Worker is required to join a busy hospital-based team, supporting adults and their families through assessment, discharge planning and access to appropriate care and support. You will work collaboratively with patients, relatives, health professionals and community services to promote independence, wellbeing and safe transitions from hospital into suitable living arrangements.

Your responsibilities will include completing strengths-based assessments, identifying eligible needs, developing person-centred support plans and coordinating packages of care, rehabilitation, reablement or accommodation where appropriate. You will undertake safeguarding enquiries, contribute to risk management, arrange reviews and ensure that decisions reflect the individual’s wishes, desired outcomes and legal rights. The role will also involve working within relevant legislation, policies and professional standards, maintaining accurate case records and preparing clear reports for meetings, panels and other professional forums.

You should hold a recognised Social Work qualification and be registered with the appropriate professional regulator, with substantial experience in adult social care, ideally within a hospital, discharge or multidisciplinary setting. A sound knowledge of the Care Act, mental capacity and safeguarding procedures is essential, together with the ability to manage a varied caseload and make balanced, evidence-based decisions in a fast-paced environment. Strong communication, negotiation and organisational skills are required, as is the confidence to work independently while contributing positively to a wider team. You will be committed to anti-discriminatory practice, able to build effective relationships with people from diverse backgrounds and focused on achieving safe, timely and person-centred outcomes.
