The London Borough of Sutton is seeking an experienced and dedicated Qualified Social Worker to join our Adult's Social Work Hospital Team. This role is essential in providing high-quality support and intervention for adults requiring social care services while they are in the hospital setting. As a Qualified Social Worker, your key responsibilities will include:
Assessing the social care needs of adults admitted to the hospital. Developing and implementing care plans in collaboration with patients, families, and multidisciplinary teams. Providing information and support to individuals and their families regarding available services and resources. Facilitating safe discharges from the hospital by coordinating with community services and ensuring continuity of care. Advocating for the rights and well-being of patients in accordance with relevant legislation and policies.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (BA/MA in Social Work, DipSW, or equivalent). Current registration with Social Work England. Significant experience in adult social work, particularly within a hospital or healthcare setting. Strong assessment and intervention skills, with the ability to work effectively in a fast-paced environment.
