We are currently seeking a dedicated and experienced Adult's Social Worker to join our Neighbourhood Team in Bolton. As a key member of the team, you will be responsible for providing high-quality social work services to adults in need within the community.

Key responsibilities and duties include:

  • Conducting assessments and developing care plans for vulnerable adults
  • Providing support and guidance to individuals and families to promote independence and well-being
  • Working collaboratively with multi-disciplinary teams and external agencies to ensure holistic care
  • Advocating for clients' rights and representing their best interests
  • Participating in case conferences, reviews, and safeguarding procedures

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with a valid HCPC registration
  • Demonstrated experience in adult social work, preferably within a neighbourhood team setting
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively under pressure and manage a complex caseload
