We are currently seeking a dedicated and experienced Qualified Social Worker to join our Transfer of Care Team in Southwark. As a key member of the team, you will be responsible for facilitating the safe and timely discharge of patients from hospitals to their homes or appropriate care settings.

Key responsibilities and duties include:

  • Conducting assessments to determine the needs of patients and their families
  • Developing and implementing care plans in collaboration with multidisciplinary teams
  • Providing support and guidance to service users to ensure a smooth transition from hospital to community-based care
  • Advocating for the needs and rights of vulnerable individuals
  • Participating in case conferences and reviews to monitor progress and adjust care plans as needed

The ideal candidate for this role will possess the following essential qualifications and skills:

  • Qualified Social Worker with current registration with Social Work England
  • Experience working in a similar role within a health or social care setting
  • Strong assessment and communication skills
  • Ability to work collaboratively with a range of professionals and agencies
  • Knowledge of relevant legislation and policies related to adult social care

In addition to the essential qualifications and skills, candidates with experience in discharge planning or working with older adults and individuals with complex needs will be highly regarded.
