We are seeking a dedicated and experienced Safeguarding/Care Planning Assistant Practitioner to join our team in Middlesbrough. In this role, you will play a crucial part in supporting the delivery of high-quality care planning and safeguarding services for vulnerable individuals and families.

Key responsibilities and duties:

  • Assist in the development and implementation of care plans for service users, ensuring their safety and well-being.
  • Conduct assessments and reviews to identify risks and protective factors, working collaboratively with other professionals.
  • Participate in case conferences and meetings, providing input and support to ensure the best outcomes for service users.
  • Maintain accurate and up-to-date records of interventions and progress, adhering to relevant policies and procedures.

Essential qualifications and skills:

  • Qualified Social Worker with a relevant degree and HCPC registration.
  • Demonstrated experience in safeguarding and care planning within a social work setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users and colleagues.
  • Excellent organizational skills and the ability to manage a demanding workload effectively.
