Your key responsibilities will include:

  • Working closely with multi-disciplinary teams, including health professionals, to coordinate effective care.
  • Maintaining accurate and up-to-date records in compliance with relevant legislation and best practices.
  • Participating in regular supervision and professional development opportunities.

To be considered for this role, you must possess a recognized social work qualification (BA/MA in Social Work or equivalent) and be registered with Social Work England. Essential skills include strong communication and interpersonal abilities, a sound understanding of safeguarding policies, and the capacity to work effectively under pressure. Experience in adult social care and knowledge of local resources will be advantageous.
